Delivery Information:
Orders of one pack of 50 forms will usually be shipped Priority Mail, and all other orders go
UPS ground. Within the Continental US, freight charges are as follows: Orders less than $75 will be billed $5.00 in freight. Orders between $75 and $100 will be billed $9.00 in freight. Orders $101 - $200 are billed $12.00 in freight, and stock orders over $200 are billed a flat $19.00, regardless of your location within the continental US. Expected delivery times are based on In-Stock items. If an item is Out-of-Stock, we will call or email you within 24 hours of your placing the order.

3- 5 Business Days for most imprinted stock items.
International Orders will be reviewed for shipping adjustments, and you will be contacted for approval if any change is necessary.

to arrange a return or exchange within 30 days of purchase. Postage is not refundable. Custom or imprinted orders, which were correctly manufactured, are non-returnable.
